How to Apply:
The application has 5 parts. Complete all 5 documents and email them to employerworkforcetrainingfund@worksystems.org by 5pm, Monday, August 28, 2006.

• Scope of Work (Word)
• Performance and Job Projections (Excel)
• Budget Worksheet (Excel)
• Budget Worksheet sample (Excel)
• Affirmation for signature (Excel)

Review the Terms and Conditions (pdf) document and email employerworkforcetrainingfund@worksystems.org with any questions.

Due Date:
Applications are due by 5pm, Monday, August 28, 2006.

All applications must be submitted electronically as Word or Excel documents.

Evaluation:
An Evaluation Team will review all applications and identify projects that best meet the goals, objectives, and targets of the program.

Notification:
The contact person on the cover of the application will be notified of the status of the application on or before September 14, 2006
